Canadaからの報告。（最近Canadaからの報告多い）。
ICU Ptではグルタミンの低下が予後と関係していることが報告されている。
グルタミン投与を行うと死亡率がさがるのか？ をRCTした。
Pt: 1223 critically ill Pt
I: Glutamine 0.35g/kg antioxidant(VitC + Vit E + selenium + zinc + beta carotine + Biosyn)
Glutamine only, antioxidant only, Glutamine + antioxidant
C: pracebo
O: (1st) 28 days mortality (2nd) 6 Mo mortality
Result: 28日死亡率には差なし。 ６ヶ月死亡率ではグルタミン投与群の方が悪い。
このtrialはICU入室後直ちに投与開始していること、Shockの患者が多く含まれている
ことがprevious studyとの違い。
「効かないのか〜」ではなく、なぜ効かないのか、Glutamine はそもそも何をしているのか？注目していきたい。
